Once an intake is completed, the Case Manager and client work together to develop an Individualized Program Plan (IPP).
An IPP is a person-centered plan that identifies the client's unique strengths, needs, barriers, and goals. It outlines the specific services, resources, funding, and support necessary to help the individual achieve greater stability, independence, and overall well-being.
The IPP process is collaborative and brings together the client, Case Manager, and other members of the client's support network, when appropriate, to create a personalized roadmap for success. Goals and action steps are tailored to the individual's circumstances and may include areas such as housing, employment, education, healthcare, mental health services, financial stability, and life skills development.
The IPP is a living document that is reviewed and updated regularly to reflect the client's progress, changing needs, and achievements, ensuring that services remain responsive and focused on helping the individual reach their goals.
